Mahopac Dental Care participates with the Guardian Dental plan. This dental plan has a contracted fee schedule and a co-payment will usually apply. Our staff will happily assist you with your dental claim forms.
We also work with many other insurance plans like Anthem Empire BlueCross Blue Shield (BCBS), Sun Life, POMCO, SIDS, United Concordia, Assurant, and many others. These plans will have a co-payment but no contracted fee schedule.
